What Is a Hairline Transplant?
A hairline transplant is a surgical hair restoration procedure that moves healthy hair follicles from a donor area (the back and sides of your scalp) to the frontal hairline, where recession has caused thinning or loss.
Key characteristics:
- Focuses on the front 1 to 2 cm of the scalp
- Uses 1,500 to 3,000 grafts on average
- Performed in a single session or two phased sessions
- Results are permanent because donor hair is genetically resistant to DHT
- Designed to look completely natural, with single-hair grafts at the front line
The goal is not just to fill in hair, but to design a hairline that matches your face shape, age, and ethnicity. A 25-year-old’s hairline should look different from a 45-year-old’s. Hairline design is the most artistic part of the procedure.
How the UHDHT Method Works for Hairline Restoration
At Assure Clinic, hairline transplants use the UHDHT (Ultra High Density Hair Transplant) method, which combines two distinct techniques.
UFME (Ultra Fine Micro Extraction)
UFME uses a specially calibrated 0.7 to 0.8 mm punch to extract follicular units from the donor area. The ultra-fine punch:
- Leaves scars smaller than 0.8 mm (practically invisible)
- Preserves donor area density for future sessions if needed
- Allows fine precision for selecting only the highest-quality grafts
Traditional FUE uses punches of 1 mm or larger, which cause more visible scarring and reduce donor area yield.
DSHI (Direct Simultaneous Hair Implantation)
DSHI implants the extracted grafts directly into the recipient area without a storage phase. This:
- Maximises graft survival (95% at Assure Clinic)
- Reduces time out of body for each follicle to under 10 minutes
- Enables ultra-high density placement (40 to 50 grafts per cm²) for a natural, full hairline
Together, UFME and DSHI form the UHDHT method, which is especially suited for hairline work because the front of the hairline demands the highest density and finest finish.
Who Is a Candidate for a Hairline Transplant?
A hairline transplant is ideal for:
- Norwood Stage 2 to 3: Receding at the temples or hairline with thinning frontal coverage
- Norwood Stage 3 Vertex: Frontal recession plus early crown thinning (combined approach)
- Post-medication plateau: Hair loss has stabilised after 6 to 12 months of finasteride or minoxidil
- Mature adults: Generally age 25+ (hair loss pattern stabilised enough to plan a long-term hairline)
- Healthy donor area: Sufficient density at the back and sides
- Realistic expectations: Wanting a natural, age-appropriate hairline rather than the adolescent hairline
Who is NOT a candidate yet:
- Active rapid hair loss without medical stabilisation
- Diffuse thinning (Ludwig-type pattern in women) may need a different approach
- Very advanced stages (Norwood 6 to 7) with limited donor supply (may need combined scalp and body hair)
- Patients under 22 to 23 years (hair loss pattern not yet established)
Our Medical Consultants conduct the first assessment. If surgery is indicated, you meet a qualified doctor who maps your Norwood stage, assesses donor density, and designs your hairline before any procedure is planned.
Hairline Transplant vs Full Hair Transplant
| Feature | Hairline Transplant | Full Hair Transplant |
|---|---|---|
| Focus area | Front 1 to 2 cm of scalp | Front + mid-scalp + crown |
| Grafts needed | 1,500 to 3,000 | 3,500 to 6,000+ |
| Sessions | 1 session, occasionally 2 | Usually 1 to 3 phased sessions |
| Duration | 4 to 6 hours | 6 to 10 hours per session |
| Recovery | 7 to 10 days visible redness | 10 to 14 days |
| Cost range | Lower total cost | Higher total cost |
| Best for | Norwood 2 to 3 | Norwood 4+ |
A hairline transplant is often the right starting point for men at Stages 2 to 3. As hair loss progresses, a second phased session can address the mid-scalp and crown later. This phased approach preserves the donor area.
The Hairline Design Process
Hairline design is the most important part of a hairline transplant. A well-designed hairline looks natural and frames the face. A poorly designed one looks artificial and creates the “doll hair” look associated with older transplant methods.
At Assure Clinic, hairline design follows five principles.
1. Age-appropriate placement
- 20s and 30s: Slightly mature placement, not adolescent
- 40s and above: Mature placement with softer front line
- Never attempt to recreate a teenage hairline on a 45-year-old
2. Temple integration
- Temples taper naturally downward and inward
- Sharp corners look artificial
- Curves and irregular edges mimic natural growth
3. Single-hair grafts at the front line
- The very front row uses only single-hair follicles
- Two- and three-hair grafts are placed behind the front line
- This creates a soft, natural gradient
4. Directional angle
- Hair at the front should grow forward at a 30 to 45 degree angle
- Hair behind the front line angles slightly upward and outward
- Incorrect angles cause visible inconsistency
5. Density planning
- Front third: ultra-high density (45 to 50 grafts/cm²)
- Back half of hairline zone: standard density (35 to 40 grafts/cm²)
- Matches natural hair distribution
Hairline design at Assure Clinic is done by qualified doctors during consultation. You see the proposed hairline marked on your scalp, discuss adjustments, and confirm before surgery.

Is a Hairline Transplant Permanent?
Yes. The transplanted follicles come from the back and sides of the scalp, a region called the “donor-safe zone”, where hair is genetically resistant to DHT (dihydrotestosterone), the hormone that causes pattern baldness.
Once transplanted, these follicles keep their genetic programming. They grow for life, just as they would have at the back of the head.
Important note: A hairline transplant restores lost hair, but it does not prevent further hair loss in untransplanted areas. Patients who stop taking prescribed medication may see continued recession elsewhere. Most patients continue a low dose of finasteride or minoxidil to preserve surrounding native hair. See our hair fall control guide for the full maintenance protocol.
Temple Hair Transplant: The Forgotten Restoration
Temple recession is the first and most common sign of male pattern baldness. A temple-only transplant is a focused subset of hairline restoration using 500 to 1,200 grafts to rebuild just the temple corners.
This is ideal for:
- Men in their late 20s with only temple recession
- Patients who want minimal intervention with maximum visual impact
- First-time transplant patients
Temple transplant is often combined with a full frontal hairline rebuild as part of the same session when recession has progressed to a full M-shape.
Forehead Reduction Surgery vs Hairline Transplant
A forehead reduction surgery (also called hairline lowering surgery) physically pulls the scalp forward and stitches it to a lower position on the forehead. This is a very different procedure from a hairline transplant.
| Feature | Forehead Reduction Surgery | Hairline Transplant |
|---|---|---|
| Method | Cuts and moves scalp skin | Moves individual hair follicles |
| Result | Entire hairline moves as a block | Natural, irregular hairline shape |
| Best for | Naturally high hairlines (not hair loss) | Receding hairlines (hair loss) |
| Scarring | Horizontal scar along hairline | Invisible micro-dots in donor area |
| Downtime | 2 to 3 weeks visible scarring | 7 to 10 days |
For men and women with hair loss, a hairline transplant is almost always the better option. Forehead reduction is usually reserved for people born with very high hairlines who have never had hair loss.
Women’s Hairline Transplant
Hairline transplants for women are different from men’s. Women typically retain the frontal hairline but experience diffuse thinning or, in some cases, a naturally high hairline they want to lower.
At Assure Clinic, women’s hairline procedures address:
- Naturally high hairlines being lowered
- Traction alopecia from tight hairstyles
- Post-surgery or scarring repair
For women’s pattern hair loss, see our dedicated Female Pattern Baldness guide, or read about our no-shave hair transplant for women.

How much does a hairline transplant cost in India?
Hairline transplant cost at Assure Clinic depends on the number of grafts required. For mild to moderate recession needing 1,500 to 2,500 grafts, pricing starts from our affordable Full Head Results tier. Full pricing is discussed during the free consultation and depends on individual assessment.
How long does a hairline transplant last?
A hairline transplant is permanent. The transplanted hair comes from the donor-safe zone and is genetically resistant to DHT, so it grows for life.
How long does it take to see results from a hairline transplant?
First new hair appears at 3 to 4 months. 50 to 60 percent of the final result is visible by 6 months. Full results take 9 to 12 months.
Is a hairline transplant painful?
No. The procedure is performed under local anaesthesia, so you feel no pain during surgery. Some patients describe mild discomfort during the anaesthesia injections. Post-procedure, mild soreness for 24 to 48 hours is managed with standard pain relief.
How many grafts do I need for a hairline transplant?
Most hairline transplants use 1,500 to 3,000 grafts. Stage 2 recession typically needs 1,500 to 2,000. Stage 3 with an M-shape needs 2,000 to 2,500. Stage 3 Vertex with frontal density loss may need 2,500 to 3,000. A qualified doctor measures the exact number during consultation.
Can I restore just my temples without doing the full hairline?
Yes. A temple-focused transplant uses 500 to 1,200 grafts to rebuild only the temple corners. This works for early recession where the central hairline is still intact.
What age is best for a hairline transplant?
Generally 25 and above. Before age 25, hair loss patterns have not stabilised, making long-term hairline design difficult. Younger patients are usually advised to start medication and monitor for 1 to 2 years first.
Will people know I had a hairline transplant?
When done correctly with the UHDHT method, a hairline transplant is undetectable. Single-hair grafts at the front line, natural irregularity, and age-appropriate design create a result that looks like your own natural hair.
What is the success rate of a hairline transplant at Assure Clinic?
95 percent graft survival rate across 20,000+ procedures. All procedures are performed by qualified doctors using the UHDHT method, with graft warranty on every procedure.
